Responsibilities

• Conduct direct outreach to initiate patient engagement and enrollment.
• Schedule initial appointments with clinicians and assist with barriers.
• Review and resolve barriers to program completion and ensure follow-up.
• Maintain KPI dashboards with Medical Director on a recurring basis.
• Collaborate with third party vendors to arrange devices and platform access.
