Job summary

Church at the Park is seeking a compassionate Case Manager to work with individuals experiencing homelessness in a shelter setting, helping them move toward stable housing. You will collaborate with community partners to coordinate services, assist with housing readiness, and connect participants to resources such as OHP and behavioral health services.

The role emphasizes data accuracy, HIPAA compliance, and coordination with medical and social service providers.

Qualifications

• Associate degree preferred in human services, social work, or sociology.
• Minimum two years in case management or social services.
• Experience mediating between individuals exhibiting escalated behavior.
• Ability to collaborate with case managers, healthcare providers, and social service agencies.
• Experience with homelessness, chronic illness, behavioral health, or substance use is a plus.
• Strong organizational skills to manage multiple participant needs.

Responsibilities

• Maintain thorough, accurate, and timely documentation of participant interactions and progress.
• Submit required data, case notes, and reports per program, organizational, and grant requirements.
• Ensure PHI handling complies with HIPAA and confidentiality standards.
• Participate in training and professional development to strengthen team capacity.
• Coordinate medication pickup and delivery for participants, ensuring proper receipts and forms.
• Connect participants to resources such as OHP, domestic violence services, substance use treatment, and behavioral health services.
