Qualifications

• Bachelor’s degree in human services or related field required.
• 6 months to 1 year housing or case management experience preferred.
• Ability to work with diverse populations and external agencies.
• Strong communication and organizational skills; proficient with Excel.

Responsibilities

• Provide housing stabilization services to participants.
• Complete intakes, treatment plans, and documentation for housing services.
• Open Housing Episodes in Epic and maintain participant medical records.
• Coordinate referrals for supportive services and track engagement.
• Update census, track hours, and prepare quick notes for services.
• Respond to calls, manage messages, and ensure proper releases.
• Attend HSP meetings and assist with workflow and billing.
