Job summary

The Providence Center Inc. is seeking a Housing Stabilization Case Manager to provide housing stabilization services and case management support within the housing department.

You will perform intake, develop treatment plans, maintain participant data in Excel, and coordinate referrals with outside agencies. Responsibilities include documenting services, creating and maintaining records in EPIC, and collaborating with staff to meet monthly target measures while maintaining strong communication

Qualifications

• Bachelor's degree required in Human Services or a related field.
• Minimum 6 months to 1 year of housing or case management experience.
• Ability to work with a diverse population and interact with clients and external agencies.
• Strong organizational and communication skills required.
• Proficiency with Excel to create spreadsheets with formulas.

Responsibilities

• Collaborate with supervisor on scheduling interactions with recipients to meet monthly target measures.
• Open Housing Episodes in Epic to ensure participants have a medical record number and to document services provided.
• Respond to inquiries from internal/external stakeholders and refer to appropriate staff or resources.
• Assist with intakes, treatment plans, and housing stabilization services for participants.
• Update census and track hours of service for each participant; complete notes for services provided.
• Coordinate referrals to outside agencies and ensure releases are completed and valid for participants.
