Job summary

Carolinas CARE Partnership is hiring a Housing Case Manager in Charlotte to support clients in securing permanent housing and building economic and social-emotional self-sufficiency. You will conduct assessments, create individualized plans, link to resources, and manage a 15–20 client caseload.

The role emphasizes trauma-informed, client-centered services, collaboration with the Housing Manager and Behavioral Health Team, and ongoing outreach to engage clients and promote well-being.

Qualifications

• Bachelor’s degree in social work and 2 years of case management with chronically homeless adults.
• MSW with 1 year of case management experience with chronically homeless adults.
• Valid driver’s license and reliable transportation.
• Ability to read leases, medical records and court documents; strong writing and reporting skills.

Responsibilities

• Provide case management and service coordination to assist clients in achieving housing stability and health.
• Develop and implement individualized care plans with clients, addressing daily living skills and employment linkage.
• Link clients to community resources, conduct home visits, and manage a caseload of 15–20 clients.
• Maintain confidentiality and document case management notes clearly and accurately.
