Qualifications

• Bachelor’s or master’s degree in a human services field or commensurate experience.
• Minimum of three years case management experience with adults who experienced chronic homelessness.
• Experience with harm reduction principles and motivational interviewing.
• Knowledge of Charlotte-Mecklenburg welfare and housing support systems.

Responsibilities

• Provide case management services to help participants transition from chronic homelessness to housing.
• Manage a caseload of approximately 24 individuals.
• Conduct visits with participants several times a week during initial months.
• Assist clients with interactions with landlords and neighbors to promote timely rent payments.
• Assess medical needs and link clients to appropriate primary and specialty care; follow up on treatments.
