Qualifications

• Bachelor’s degree in human services, psychology, counseling or related field.
• One year of experience in a behavioral health setting with case management.
• Proficient use of EMR, computer, telehealth systems, and mobile devices.
• Care coordination skills to link to resources and services.
• Reliable transportation to commute to sites as required.

Responsibilities

• Carry a caseload of 25-30 clients with weekly visits initially.
• Provide care coordination and referrals for continuity of services.
• Deliver case management and skills training aligned with treatment plans.
• Gather information from clients and support networks for appropriate care.
• Recognize crises and notify supervisory or clinical contacts.
• Document all contacts within 24 hours to agency standards.
• Participate in weekly team meetings to discuss treatment needs.
• Maintain productivity standard of 40 billable hours per month.
• Engage with program management for growth and performance review.
