Job summary

Clear Destination Inc. is seeking a Social Service Specialist to help individuals experiencing homelessness or housing instability secure and sustain housing through intensive, person-centered navigation and tenancy support services.

The role involves assessments, housing plans, and coordinating with La Crosse County Human Services, landlords, and providers to ensure successful housing outcomes and ongoing tenancy stability.

Qualifications

• High school diploma or GED is required.
• Associate’s degree in Human Services or related field preferred.
• Experience in housing navigation or homelessness services preferred.

Responsibilities

• Conduct comprehensive intakes and housing-focused assessments to identify barriers to obtaining housing.
• Develop and implement person-centered housing plans with measurable goals.
• Assist participants in locating, applying for, and securing housing, including housing searches and applications.
• Coordinate with landlords and partners to identify housing opportunities and advocate for participants.
• Maintain compliant documentation for Medicaid and housing services.

Education

High school diploma or GED

Associate’s degree in Human Services or related field

Peer Support Specialist certification (preferred)
