Care For the Homeless in New York, NY is seeking a Housing Specialist to assist clients in securing and maintaining stable housing, conduct needs assessments, and develop individualized housing plans with the support of case managers. You will work with landlords and service providers, coordinate resources, and ensure HUD compliance while documenting in EHR systems.

Formación

• Bachelor's degree in Social Work, Public Health, or related field preferred.
• Minimum of 2 years of experience in housing services or case management.
• Experience with HUD regulations and HUD-funded programs preferred.
• Proficiency with EHR or housing management software.
• Excellent organizational and communication skills; ability to work with diverse populations.
• Bilingual in English and Spanish strongly preferred.

Responsabilidades

• Conduct comprehensive housing needs assessments and document findings.
• Develop individualized housing plans with clients in collaboration with case managers and social workers.
• Provide ongoing housing support and coordinate with landlords and service providers.
• Maintain accurate records and ensure HUD grant compliance.
