The Women’s Center of Wake County seeks a Housing Navigator to provide strengths-based, client-centered support to individuals facing housing instability. You will coordinate housing placements, connect clients to services, advocate within housing systems, and work with case managers, legal aid partners, and community health workers to promote housing justice.

Qualifications

• Bachelor’s degree in social work, psychology, or a related field, or equivalent direct-service experience.
• 0–3 years of experience in housing services, case management, or behavioral health.
• Strong knowledge of Wake County housing programs, resources, and navigation.
• Proficiency with HMIS and data entry; commitment to housing-first approaches.
• Maintains confidentiality per HIPAA and ethical standards.

Responsabilités

• Conduct intake assessments to identify housing history, barriers, and goals.
• Develop individualized housing stability plans with each client.
• Assist clients in locating and applying for affordable housing and housing programs.
• Provide documentation support and connect clients to financial resources.
• Follow up with clients per their service plans and provide wrap-around case management.
• Engage landlords and community partners to expand housing options.
• Maintain confidential case notes in HMIS and track outcomes.
• Coordinate referrals to partner agencies and participate in coordinated entry systems.
