Job summary

Lutheran Social Services of Southern California is seeking a Housing Navigator to support individuals in achieving stable housing. The role focuses on person-centered housing transitions, education, and tenancy sustainment across multiple program components, complying with HCBS requirements.

You will document services in the EHR, coordinate with the Program Manager, and engage with community resources to promote housing stability.

Qualifications

Experience in housing services or case management.
Understanding of IDD populations and person-centered practices.
Ability to work independently and with teams.
Ability to follow Live Scan and background check requirements.

Responsibilities

Assist Program Manager in developing and managing housing and counseling program, including training and advocacy.
Plan and create housing-related workshops and training activities aligned with PCP.
Present and facilitate workshops on housing access, tenancy skills, and shared living education.
Identify housing resources such as move-in assistance, utilities setup, and relocation logistics.
Provide direct housing navigation services: housing search, application help, lease-up coordination, tenancy support.
Develop and monitor individualized housing support plans and document progress in the EHR.
