Job summary

Victory Programs Incorporated in Dorchester, MA, is seeking a Housing Case Manager to coordinate and deliver high-quality direct case management services for homeless families at Chamblet and Revision Family Homes. The role reports to the Program Director for weekly supervision and assignments.

The position involves housing search support, referrals, and collaboration with the Stabilization Case Manager to plan aftercare and secure housing stability for residents.

Qualifications

• Bachelor degree or 2 years of experience preferred, or high school diploma with 3+ years of client advocacy experience.
• Experience advocating for clients with housing authorities, managers, or landlords is preferred.
• Massachusetts driver’s license with a safe driving record.
• CPR certification (may be obtained upon hire) and TB screening (may be obtained upon hire).
• Three strong work-related professional references; at least 2 from manager or supervisor.

Responsibilities

• Provide weekly collaborative case management for 14 families addressing housing, finances, vocation and education, health, childcare, and legal issues.
• Assist families with housing search, referrals, and applications.
• Transport clients using agency vehicles to housing, legal, and medical appointments as needed.
• Develop and maintain service linkages with local providers and housing partners.
• Document services in HMIS/ETO and residents’ case files.
