Job summary

Victory Programs Incorporated is hiring a Housing Stabilization Case Manager in Boston, MA. The role provides 24-month post-shelter stabilization support, coordinating housing retention, assessments, referrals, and documentation.

On-site work with a focus on homeless families and pregnant women, reporting to Program Director. The position requires a Bachelor's degree (or related experience), MA driver's license, CPR/First Aid certification (or obtained on hire), and strong communication skills.

Qualifications

• Bachelor degree with 1-2 years’ experience preferred, or High school diploma with 3 or more years of client advocacy experience.
• Experience housing search, stabilization and retention, case management, SUD services, employment training and job search advised.
• Valid MA driver’s license and safe driving record.
• Proficient computer skills, including MS Word and Excel.

Responsibilities

• Coordinate and deliver direct case management services to residents who recently moved from shelter to permanent housing.
• Provide monthly case management including housing retention support, assessment, planning, reassessment, referrals, and housing applications.
• Transport and accompany clients to housing, legal, and medical appointments using agency vehicles.
• Develop, implement, and facilitate groups and workshops as required by funder and agency.
• Maintain HMIS data entry and timely reporting of housing assessments.
