Qualifications

• Must have the ability to reason, effectively communicate, and motivate individuals experiencing homelessness, mental health and substance use challenges.
• Prior experience in housing services or case management is a plus.
• Strong data entry and organizational skills are required.

Responsibilities

• Manage and support permanent supportive housing participants.
• Perform intake assessments and coordinate referrals to case managers and community resources.
• Accurately capture and enter data in a timely and accurate manner.
• Coordinate with partners to ensure participant needs are met.
