STAFF-PUBLIC HEALTH FUND is seeking a full-time Supportive Services Case Manager in Richmond, VA. The role focuses on assessing client needs, creating service plans, and coordinating resources across housing, healthcare, and employment services to promote stability and independence.

The position requires compassionate, organized professionals with strong communication skills and the ability to handle multiple cases while maintaining confidentiality.

Qualifications

• 1+ year of case management or supportive services experience.
• Strong knowledge of local Richmond, VA community resources and social service systems.
• Excellent communication and interpersonal skills with diverse client populations.
• Ability to manage multiple cases while maintaining confidentiality and attention to detail.

Responsibilities

• Conduct intake assessments to evaluate clients' needs and strengths.
• Develop individualized service plans with clients and stakeholders.
• Coordinate with housing, healthcare, employment services, and counseling resources.
• Maintain accurate case records and document interactions and progress.
• Advocate with service providers and agencies to reduce barriers to access.
• Provide crisis intervention and support as needed.
• Monitor client progress and adjust plans accordingly.
• Attend multidisciplinary meetings and contribute to program improvements.
