Job summary

Pacific Health Group in Monterey County seeks a field-based Housing Navigator to support CalAIM Community Supports Housing programs and housing deposits, conducting intake assessments, and building partnerships with housing providers. This role prioritizes independent work, compassionate outreach, and timely documentation of services.

You will work in the community, travel locally, and coordinate with healthcare and social services to achieve housing stability for high-need members.

Qualifications

• Strong understanding of housing system and local housing resources, housing navigation, and community-based support programs.
• 3-5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Proficient with technology including EHR and case management platforms.

Responsibilities

• Support Housing Transition Navigation Services for members experiencing homelessness or at risk.
• Coordinate Housing Tenancy & Sustaining Services to maintain stable housing.
• Assist with Housing Deposits and required documentation for deposits.
• Collaborate with housing authorities, landlords, shelters, and providers to place and sustain housing.
• Maintain accurate, real-time documentation of client interactions following regulatory guidelines.
