Job summary

Pacific Health Group is seeking a field-based Housing Stability Specialist to support CalAIM community housing programs. You will conduct outreach, intake assessments, and coordinate housing resources with providers, shelters, landlords, and community organizations.

Expect significant travel within the county and surrounding areas. The role emphasizes independent work, strong time management, and compassionate, client-centered care to improve housing access and health outcomes for high-needs

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs preferred.
• Strong communication, empathy and time management.

Responsibilities

• Provide housing stability services and navigation to housing resources.
• Coordinate care plans, appointments, and long-term housing support.
• Engage with members in field settings and maintain timely documentation.
• Build partnerships with housing providers, shelters, and community organizations.
• Monitor housing stability and address barriers affecting care.
