Job summary

Pacific Health Group is seeking a field-based Housing Stability Specialist to support CalAIM Community Supports Housing programs, including Housing Transition Navigation Services. You will conduct intake assessments, build partnerships with housing providers, and guide members toward stable housing with comprehensive, person-centered care.

In this role you will manage housing deposits, coordinate tenure and sustaining services, and monitor outcomes while visiting in shelters, homes, and clinics

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Strong understanding of Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Excellent communication, empathy, and cultural competence.
• Proficient with technology, including case management systems and documentation tools.
• Ability to document in real-time and manage member data accurately.

Responsibilities

• Support housing stability for members through Housing Transition Navigation and related services.
• Coordinate housing tenancy and sustaining services with landlords and providers.
• Assist with Housing Deposits, organizing required documentation and access to deposits.
• Collaborate with authorities, shelters, and community-based organizations for housing placement.
• Develop individualized care plans and coordinate services and follow-ups.
• Maintain timely documentation and ensure compliance with program requirements.
