Pacific Health Group in California seeks a field-based Housing Stability Specialist to support CalAIM Community Supports housing programs, conduct intake assessments, and build partnerships with housing providers.

You will manage care plans, coordinate deposits, housing transitions, tenancy sustainment, and accompany members to appointments while ensuring timely, person-centered service and strict documentation.

Qualifications

• 3–5 years in case management, social services, or healthcare
• Experience with Medi-Cal, CalAIM, and Community Supports programs preferred
• Strong understanding of housing systems and housing resources
• Excellent communication, empathy, and cultural competence
• Proficiency with real-time documentation and multiple systems

Responsibilities

• Support housing program participants through Housing Transition Navigation Services
• Coordinate Housing Tenancy & Sustaining Services
• Assist with Housing Deposits and deposit assistance coordination
• Liaise with housing authorities, landlords, shelters, and community organizations
• Conduct field visits and housing-related assessments
• Document in real-time and maintain accurate records
