Job summary

Pacific Health Group is seeking a Field-Based Housing Support Specialist to empower Medi-Cal members through Housing Transition Navigation, tenancy safeguarding, and deposit assistance.

You will conduct intake assessments, coordinate with housing authorities, landlords, and community providers, and deliver person-centered care in the community. A strong focus on outcomes, documentation, and collaboration with interdisciplinary teams is essential.

Qualifications

• 3-5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs preferred.
• Proficient in case management systems, EHRs, and real-time documentation.
• Strong organizational and time-management skills.
• Excellent communication and cultural competence.

Responsibilities

• Provide Housing Stability Services, including Housing Transition Navigation and Housing Deposits.
• Coordinate with housing authorities, landlords, shelters, and healthcare providers.
• Deliver care management and coordinate services for housing stability.
• Engage with community outreach and build referral pathways.
• Maintain accurate documentation and comply with program requirements.
