Job summary

Catholic Charities Archdiocese of Galveston-Houston is seeking a Case Navigator to assist at-risk individuals and families in stabilizing housing. You will assess needs, coordinate supportive services, and guide clients through housing resources with empathy and advocacy.

The role requires a bachelor’s degree in social work or related field, 2 years of case management in homelessness services, and a valid Texas driver’s license.

Qualifications

• Bachelor degree in social work, psychology or related field required.
• Two (2) years of case management experience within the homelessness services, housing programs, or social service.
• One (1) year of case management experience preferred
• NSPIRE Inspector Training (provided by agency- must pass within first 90 days of employment)
• Valid Texas Driver's License

Responsibilities

• Assess clients’ needs and coordinate housing resources and supportive services.
• Maintain case files and document services in program databases.
• Meet clients twice monthly to review progress and adjust goals as needed.
• Provide guidance and advocacy to help clients achieve housing stability.
• Record data and prepare reports to track program outcomes and meet funder requirements.
