Job summary

Catholic Charities of the Archdiocese of Galveston-Houston is seeking a skilled program supervisor to oversee wrap-around case management for pregnant women, single parents, and families in the Houston area. You will lead the program's case managers, coordinate services across health, housing, education, and employment, and ensure compassionate, culturally competent support.

This role requires an associate degree, CHW certification or within 6 months, and bilingual English/Spanish.

Qualifications

• Associate degree in social work, psychology, or related field.
• Community Health Worker Certification or obtain within first 6 months of hire.
• Bilingual in English and Spanish.

Responsibilities

• Provide comprehensive wrap-around case management services to pregnant women, single parents, and families, including intake assessments and ongoing support.
• Lead program case managers; provide guidance, support, and mentorship.
• Collaborate with program leadership to develop policies and best practices.
• Provide support for pregnancy-related challenges, including prenatal care, childbirth, parenting, and family dynamics.
• Coordinate with Counseling program for group sessions and access to mental health and other supports.
• Coordinate prenatal to postnatal care, appointments, transportation, and communication among healthcare providers.
• Help clients access health insurance, Medicaid, and other financial assistance programs.
• Connect clients to housing, food, childcare subsidies, and utility assistance.
• Conduct outreach to community agencies and service providers to expand referrals and access resources.
• Facilitate life skills, personal finance, parenting, and job training classes; support GED and vocational training opportunities.
• Perform other assigned duties.
