Job summary

Lighthouse MI is seeking a Case Manager to support formerly homeless individuals and families in Lighthouse’s permanent supportive housing program at Coolidge Place Apartments in Oak Park. You will coordinate wraparound services, connect clients with community resources, and collaborate with a team of case managers to address housing stability.

The role emphasizes strong interpersonal skills, data entry in HMIS, and partnership with landlords to facilitate housing moves and long-term stability.

Qualifications

• Lived or professional experience working with people experiencing homelessness or extreme poverty in the US.
• Bachelor's degree
• 1–2 years’ experience in a role related to human/social services or case management
• Reliable transportation and automobile insurance, plus a valid driver’s license
• Proficiency with Microsoft Office Suite and Google Apps
• Ability to multitask and work with a diverse population

Responsibilities

• Maintain a caseload of 16 clients
• Assist clients in obtaining personal documents for housing programs
• Outreach to landlords for mediation, negotiation, and tenant relations
• Link clients to resources for move-in costs, furniture, moving services, etc.
• Provide ongoing case management following the Housing First model
• Enter data in HMIS and internal tracking-log within 48 hours
• Screen applications and coordinate with the Property Manager
• Conduct monthly general inspections and support housing stability
