Job summary

Lighthouse MI is hiring two Supportive Housing Case Managers to assist formerly homeless individuals and families in securing longer-term housing solutions in the Southfield, MI area. The role involves wrap-around services, housing navigation, and collaboration with landlords and community partners.

Ideal candidates have experience with homelessness, strong writing skills, and proficiency in MS Office and Google Apps.

Qualifications

• Experience living or working with people experiencing homelessness or extreme poverty in the US.
• Minimum 1 year of experience in a role related to human/social services and/or case management.
• Bachelor's degree, or five years of experience within a related role.
• Reliable transportation and maintenance of automobile insurance.
• Valid driver’s license; Lighthouse reserves the right to require a driving record check when driving is essential.
• Knowledgeable in Microsoft Office Suite and Google Apps.
• Flexibility and the capacity to prioritize tasks in a rapidly changing environment.
• Ability to multitask effectively in a fast-paced environment.
• Ability to work with a challenging and diverse population.

Responsibilities

• Provide wrap-around case management services to clients following the Housing First model.
• Screen applications and connect eligible community members to Permanent Supportive Housing opportunities.
• Maintain a caseload of clients and assist with housing program documentation.
• Outreach to landlords for mediation, negotiation, and tenant relations.
• Link clients to move-in costs, furniture, moving services, etc.
• Enter data in HMIS and internal tracking logs, and ensure timely program entries.
• Coordinate with Property Managers and conduct housing inspections.
• Assist with answering phones and crisis calls as needed.
• Participate in task forces and committees supporting the homeless population.
