Lighthouse MI is a growing nonprofit in Michigan seeking two Supportive Housing Case Managers to support formerly homeless and at‑risk individuals and families placed Wellspring Recovery Homes. The roles are based in Southfield with on‑site work across the surrounding communities.

You will provide wrap‑around services, help clients obtain documents, screen for MSHDA vouchers, maintain a 15‑client caseload, coordinate with landlords and housing partners, and document case notes in HMIS following

Qualifications

• 1 year of experience in human/social services and/or case management
• Bachelor's degree or five years of related experience
• Reliable transportation and automobile insurance
• Valid driver's license; driving record check may be required
• Knowledgeable in Microsoft Office Suite and Google Apps
• Ability to multitask in a fast-paced environment
• Experience with homeless populations a plus

Responsibilities

• Screen applications and connect eligible community members to Permanent Supportive Housing opportunities
• Screen eligible community members for MSHDA Section 8 vouchers and manage MSHDA waitlist/portal
• Maintain a caseload of 15 clients
• Assist clients in obtaining personal documents for housing programs
• Outreach to landlords for mediation and tenant relations
• Provide ongoing case management following Housing First model with monthly in-home/in-community visits
• Complete timely program entries, exits, and notes in HMIS and internal logs
• Enter data in real-time or within 48 hours
• Coordinate with Property Managers and conduct monthly housing inspections
