Job summary

PATH is seeking a dedicated Case Manager for the Metro Villas site in Los Angeles, CA. You will support clients through housing stability, healthcare access, and social services, applying a trauma-informed, person-centered approach in a permanent supportive housing setting.

You will assess needs, develop ISP plans, coordinate with partners, and ensure documentation compliance while promoting dignity and housing outcomes.

Qualifications

• Two years of experience in healthcare, case management, or homeless services.
• Must have employment eligibility verification.
• CPR/First Aid training or ability to obtain.
• Background screening and drug test required.

Responsibilities

• Assist clients through all homelessness phases with a 'whatever it takes' approach.
• Help clients access housing, healthcare, and social services.
• Develop individualized service plans (ISP) addressing housing and income goals.
• Maintain client files and ensure confidentiality per HIPAA.
• Provide crisis intervention and risk assessment as needed.
• Coordinate with CES and community resources.
