Catholic Community Services (CCS) seeks a Housing Stability Case Manager in Tacoma to support residents in permanent supportive housing. You will address barriers such as behavioral health needs, medical issues, and limited income through a housing-first, trauma-informed approach.

Responsibilities include assessments, housing stability planning, resource coordination, and collaboration with property management and clinical teams. Strong communication and documentation skills are essential.

Qualifications

• Minimum 2-year experience as Case Manager with high-need populations.
• Associates Degree in Social Services or related field with 1-year experience in the field.
• Background checks and three references required.
• Reliable transportation and valid driver’s license with current insurance.

Responsibilities

• Conduct comprehensive assessments and develop Housing Stability Plans with measurable goals.
• Provide ongoing, individualized case management in permanent supportive housing.
• Assist clients in accessing healthcare, mental health services, benefits, and employment services.
• Maintain confidential client records in compliance with requirements and document in HMIS.
• Collaborate with internal teams and external partners; participate in case conferences.
• Provide trauma-informed crisis intervention and de-escalation when needed.
