Catholic Community Services Nativity House Apartments in Tacoma seeks a Housing Stability Case Manager to deliver intensive, client-centered services to people experiencing chronic homelessness and housing instability. Responsibilities include supporting housing stability and coordinating with partners to access needed resources.

The position uses a housing-first, trauma-informed, and strengths-based approach to promote self-sufficiency.

Qualifications

• Minimum 2-year experience as Case Manager with high-needs, low-income/no-income families who have experienced housing instability/homelessness, behavioral health challenges or similar while maintaining healthy boundaries.
• Associates Degree in Social Services, Psychology, Human Services, or a related field and 1-year experience in Human Services or Social Services performing duties aligned with the major duties and responsibilities of the role.
• Reliable transportation and valid driver’s license, automobile insurance and current auto registration.
• Must pass required background checks and provide three acceptable references.

Responsibilities

• Conduct comprehensive assessments and develop client-driven Housing Stability Plans with measurable goals.
• Provide ongoing, individualized case management to residents in permanent supportive housing programs to monitor progress towards housing stability.
• Assist clients in accessing healthcare, mental health services, substance use treatment, benefits, and employment services.
• Maintain regular contact with clients through home visits, office meetings, and field outreach per program guidelines and contract deliverable expectations.
• Conduct financial coaching sessions to help clients understand housing requirements, create and adhere to budgets, and build skills for independent living.
• Coordinate with property management to ensure safe and stable housing environments and conduct unit inspections.
