Job summary

Catholic Community Services is hiring a full-time Case Manager to support the Permanent Supportive Housing program in Everett, WA. You will work directly with clients to secure housing, assess needs, and coordinate services across health, social, and housing resources.

The role emphasizes client advocacy, collaboration with staff and partners, and maintaining accurate records in HMIS, with a focus on moving individuals from homelessness to stable housing.

Qualifications

• BA in social services or related field or AA in social services or related field with related experience.
• Direct service experience in case management or related duties.
• Knowledge of Substance Use Disorder and related signs/symptoms.

Responsibilities

• Work directly with clients on securing appropriate housing.
• Conduct face-to-face interviews with residents and assess needs to create initial service plans.
• Monitor participation in community programs and coordinate with the overall service plan.
• Facilitate access to community resources, financial assistance, medical care, and social services.
• Maintain client records and progress toward self-sufficiency.
• Provide case management services and advocacy to support housing goals.
• Collaborate with property management to uphold housing rules.
• Develop HMIS proficiency for client records and reporting.
