AID Atlanta is seeking a dedicated housing case manager to coordinate direct services for people living with HIV/AIDS. You will manage a caseload, perform eligibility assessments, and assist with housing applications and TBRA subsidies.

The role requires ongoing client education, linkage to employment and health services, and documentation in the EMR system to ensure optimal housing and medical outcomes.

Qualifications

• Bachelor’s degree in social work or related field required.
• 1–2 years of experience in social work/case management or similar field preferred.
• Experience in HIV housing or related services is a plus.

Responsabilités

• Provide direct service coordination, coaching and life skills support to people living with HIV/AIDS needing housing assistance.
• Offer site-based enhanced and intensive case management to HOPWA HINT members.
• Manage a caseload of up to 30 members.
• Conduct comprehensive assessments and intake for eligibility.
• Assist with housing applications, TBRA subsidies, rent calculations and inspections.
• Maintain weekly contact to support housing sustainability goals.
• Develop ISP and Housing Sustainability Plans for each member.
• Coordinate referrals and transportation needs; issue vouchers and assist with appointments.
• Educate members about services and rights; connect to employment and financial resources.
• Document all encounters in EMR within 24 hours.
