Housing Works is seeking a Housing Case Manager to provide comprehensive case management for residents living with HIV and co-occurring conditions. You will conduct home visits, coordinate services, and support housing stability through individualized goals.

Responsibilities include documentation, service planning, and collaboration with internal departments and community partners to connect clients with healthcare, mental health, and financial resources.

Competenze

• Bachelor’s degree in social work, psychology, counseling, or related field preferred; Master’s degree a plus.
• Minimum of three years of relevant social services experience with homelessness, mental health, and/or substance use.
• Ability to document in electronic systems and maintain client confidentiality.

Mansioni

• Provide case management, counseling, care coordination, assessment, and service plan development.
• Document all services and interventions per funder, regulatory, and internal standards.
• Perform monthly home visits and coordinate with clients to achieve housing stability.
• Coordinate with internal and external partners to connect clients with needed services.
