Job summary

The Housing Case Manager (HCM) at Housing Works provides comprehensive case management and care coordination to residents living with HIV and co-occurring conditions. You will work in clients’ homes, offices, and the community to promote independent living, financial management, and health goals.

Responsibilities include supportive counseling, care coordination, documentation, and coordinating with multiple departments to connect residents with needed services and housing stability.

Qualifications

• Minimum of three years of relevant experience in a social services setting.
• Experience working with individuals who have homelessness, mental health issues, and/or substance use.
• Bachelor's degree in social work, psychology, counseling, or related field preferred.

Responsibilities

• Provide case management services, including supportive counseling, care coordination, linkage and referral.
• Document all services and interventions per funder, regulatory and internal standards.
• Complete home visits, assessments and service plans within required timelines.
• Coordinate care with internal and external service providers and agencies.
