Housing Works, Inc. seeks a Housing Case Manager to deliver comprehensive case management and care coordination for residents living with HIV and co-occurring conditions.

You will work in clients’ homes, offices, and community settings to promote housing stability and independent living skills. Responsibilities include counseling, care coordination, documentation, and collaboration with multiple departments to connect clients with primary care, mental health, and social supports.

Qualifications

• Bachelor's degree in social work, psychology, counseling, or related field preferred; Master’s Degree a plus.
• Minimum of three years of relevant social services experience.
• Ability to document in electronic systems and maintain confidentiality.

Responsabilités

• Provide case management services, counseling, care coordination, and referrals.
• Document all services and interventions following standards.
• Support residents in housing stability and independent living skills.
• Coordinate with internal/external partners and attend case conferences.
