Job summary

Catholic Community Services and Catholic Housing Services of Western Washington is seeking a Case Manager to support Veterans and families through Rapid Rehousing efforts. You will screen referrals, assess housing needs, and help clients secure permanent housing using Housing First principles.

The role includes building Housing Stability Plans, coordinating with VA and community resources, and entering data into HMIS to track progress toward stable housing in a timely manner.

Qualifications

• Post-secondary education in a social services field with 1 year of experience preferred.
• Experience serving low-income households and connecting to housing and support services.
• Ability to work independently and in a team; strong organization and communication skills.

Responsibilities

• Receive referrals and screen for Rapid Resolution.
• Develop and implement Housing Stability Plans with clients.
• Assist with locating and securing housing and maintain HMIS data.
• Promote CCS mission and coordinate with community resources.
• Maintain client files with accuracy and timeliness.
