The role focuses on eviction risk interventions, connecting clients to services, and documenting progress.

Qualifikationen

• Current resume must include bulleted details of direct housing experience.

Aufgaben

• Conduct assessments and interviews with clients to determine eligibility.
• Develop and update action plans, case notes, and related documents.
• Document care and follow support plans with strong reading and writing communication.
• Provide ongoing life skills development to improve independence and quality of life.
• Coordinate with medical, mental health, and other services for clients.
• Refer clients to community services and assist with SSI/SSDI/Medicaid processes.
• Occasionally visit clients at home and coordinate with property management for wellness checks.
• Track outcomes and report data as directed.
• Attend internal and external meetings and travel within the area as needed.
