Job summary

Compass Career Solutions in Tacoma, WA, is seeking a housing services coordinator to conduct client assessments, develop action plans, and provide ongoing life skills support. This in-office role includes community outreach and coordination with medical and social services.

You will document care, follow support plans, and collaborate with internal and external teams to help clients access SSI, SSDI, Medicaid, Medicare, and SNAP. Some home visits and travel within the area may be required.

Qualifications

• Bachelor's Degree in social services or related field; minimum one year experience in coordinating supportive housing or independent living services.
• High School Diploma or GED with two years of relevant experience in coordinating supportive housing or independent living services.

Responsibilities

• Conduct assessments and interviews with clients to determine eligibility.
• Develop and update action plans, case notes, and related documents.
• Provide life skills development to improve independence and quality of life.
• Document care and follow support plans; collaborate with internal and external teams.
• Assist clients with SSI, SSDI, Medicaid, Medicare and other services.
