Job summary

Jamboree in Sacramento seeks a Floating Housing Case Manager to oversee programs that foster life skills, coping strategies, and housing stability for residents across multiple properties. You will provide linkage and referrals to increase stability for those transitioning from homelessness.

The role requires experience with homelessness and mental health challenges, a strong understanding of Housing First and Harm Reduction models, and the ability to work with diverse populations in a team

Qualifications

• Experience working with individuals experiencing chronic homelessness and mental health challenges.
• Ability to maintain relationships while upholding professional boundaries.
• Familiarity with Housing First, Harm Reduction and Recovery Oriented service models.

Responsibilities

• Provide supportive services to address barriers that affect progress toward goals.
• Maintain tenant rosters with entry-exits for Jamboree properties.
• Monitor for crisis signs and respond accordingly.
• Document resident participation and progress in services.
• Coordinate intake and orientation for new residents.
• Link residents with on- and off-site services and programs.
• Maintain relationships between community supports, providers, and property management.
