Job summary

Lutheran Social Services of Northern California is seeking a dedicated Case Manager to support adults and families transitioning from homelessness into stable housing in Sacramento. You will provide comprehensive assessments, develop individualized plans, and coordinate services with partners to address mental health, AOD, education, and employment.

This role requires strong writing and analytical skills, the ability to work independently, and sensitivity to diverse backgrounds.

Qualifications

• BA or BS degree from an accredited college or university, preferably with a major in a behavioral science, and/or 2-3 years experience in the human services field.
• Ability to work with adults or youth from diverse backgrounds who have histories of homelessness.
• Knowledge of Housing First and trauma-informed treatment frameworks.
• Experience with AOD, mental health, domestic violence histories, and collaboration with service providers.
• Strong writing and analytical skills.

Responsibilities

• Provide case management to families or adults from homelessness now residing in transitional housing.
• Support development of client-focused treatment plans and ongoing assessments.
• Provide crisis intervention, referrals, and collaborative consultation with service providers.
• Address critical needs including mental health, AOD, education, employment, budgeting, and parenting skills.
• Maintain accurate written case files, treatment plans, quarterly summaries, and program reports.
• Attend housing services and program meetings; guide clients in daily living skills and self-sufficiency.
• Discharge planning begins at entry and aims to avoid homelessness.
• Prepare HMIS data updates and participate in team meetings.
