Jamboree in Claremont, CA is seeking a Housing Case Manager to support residents at Larkin Place, assisting with life skills, housing stability, and linkage to on-site and off-site services.

You will maintain tenant rosters, input data into HMIS, coordinate with providers, and contribute to wellness events and education programs while aligning with Housing First and Harm Reduction principles.

Qualificações

• Over 21 years of age.
• Experience with homelessness and mental health challenges.
• Familiar with Housing First, Harm Reduction and/or Recovery Oriented service models.
• Ability to maintain boundaries while engaging with diverse communities.
• AA degree preferred; or equivalent experience serving homeless individuals.
• Ability to work in an interdisciplinary team and independently.
• Knowledge of independent living skills (cooking, budgeting, housekeeping, being a good neighbor).
• Valid California driver’s license and insured vehicle available.
• Willingness to evening/weekend hours and travel throughout California.

Responsabilidades

• Provide supportive services to address barriers toward goals.
• Maintain tenant rosters with entry-exits for properties.
• Monitor for signs of crisis and respond.
• Record keeping, documenting and reporting.
• Document resident participation and track progress of goals.
• Prepare monthly newsletters describing services and events.
• Maintain off-site services information and provide referrals.
• Provide onsite life skills training and advocacy.
• Complete intake assessments and orientation for new residents.
• Coordinate services between supports, providers and management.
• Promote community engagement and a positive atmosphere.
• Offer daily Adult Education groups/workshops onsite.
• Plan Health and Wellness events and community-building annually.
• Input data into HMIS accurately and timely.
• Travel throughout California as needed.
