Job summary

HOPICS is seeking a Bringing Families Home Case Manager in Los Angeles to coordinate housing placement and supportive services for families transitioning to permanent housing. The role requires ensuring timely HMIS data entry and maintaining housing plans with clients.

Under the Program Manager, you will handle eligibility screening, service coordination, and routine reporting while collaborating with partners and attending team meetings.

Qualifications

• Two years of job-related experience with homeless individuals and families, or five years in social services.
• Working knowledge of Microsoft Word and Excel.
• Valid driver’s license and auto insurance; reliable transportation.

Responsibilities

• Coordinate housing placement and supportive services to transition families to permanent housing.
• Update client records in HMIS and CS Care within 24 hours of eligibility determination.
• Prepare and submit weekly reports and deliverables; attend weekly case conferences.
