Job summary

The Coalition for the Homeless of Central Florida is seeking a Stabilization Case Manager to provide stabilization case management for the Rapid Rehousing program and shelter guests transitioning into permanent housing with rental assistance. The role uses a housing-focused approach, coordinating moving logistics and rental support.

The case manager helps clients develop Housing Stability Plans, build monthly budgets, and assist with applications and lease negotiations while maintaining HMIS

Qualifications

• Associate degree or equivalent in social services; bachelor’s preferred.
• 3+ years of documented case management experience.
• Experience in homeless services, case management, mental health or rapid rehousing preferred.
• Flexible schedule including evenings/weekends; valid driver’s license; pass background screening.

Responsibilities

• Maintain contact with clients and document interactions in HMIS within 24 hours.
• Develop and update Housing Stability Plans addressing barriers to housing.
• Build monthly budgets with clients to promote financial stability.
• Assist clients in identifying housing options using Housing Connector or similar resources.
• Assist with applications, fees, and lease negotiations with landlords.
• Help clients obtain household items through donations.
• Mediates tenant issues and de-escalates crises when needed.
• Provide time-limited services to stabilize clients in permanent housing.
• Connect clients to safety, well-being, and employment resources.
• Monitor housing stability and assist during rental assistance periods.
• Maintain and submit monthly documentation for reporting.
• Demonstrate teamwork and professional conduct with staff and clients.
• Maintain confidentiality and adhere to all agency policies.
• Meet newly assigned clients within one business day of assignment.
• Ensure client files are accurate and up to date.
• Gather documents and upload to HMIS; assist with missing documents.
