Job summary

Charter House Coalition is seeking a compassionate Housing Case Manager to support individuals experiencing homelessness. This in-person role offers a 40-hour work week with a Monday–Friday schedule and flexible hours to meet program needs.

You will complete shelter intakes, develop housing stability plans, and coordinate access to housing, healthcare, mental health, and benefits. The position includes on-call rotation and opportunities for professional development.

Qualifications

• Experience in case management or related field.
• Familiar with electronic documentation systems.
• Compassionate and committed to serving people experiencing homelessness.

Responsibilities

• Complete shelter intakes and comprehensive assessments.
• Develop housing stability plans for participants.
• Connect guests with housing, healthcare, and social services.
• Assist with housing searches, applications, and move‑in coordination.
• Advocate for clients via appointments and community partners.
• Maintain accurate case notes and documentation.
• Manage caseload of ~12–18 individuals with weekly meetings.
• Participate in Coordinated Entry and staff meetings.
• Support shelter operations and on-call rotation every 3–4 weeks.
