Job summary

Qualifications

• AA/AS in Public Health or related field.
• BA/BS in Public Health, Health Services Administration, Social Work, or related field preferred.
• One year of full-time housing case management or equivalent experience in a social services or mental health program serving homeless persons.

Responsibilities

• Establish and maintain a caseload and conduct Needs Assessments related to health, housing, income, and services.
• Meet weekly with clients to develop and monitor housing stability plans.
• Assist clients in obtaining identification and benefits such as Medi-Cal, SSI, GA, CalFresh.
• Coordinate referrals to Housing, Income, Mental Health, and other services.
• Maintain HMIS documentation and complete forms within 24 hours of service.
• Educate clients on daily living skills and support goal attainment for permanent housing.
• Conduct outreach to landlords and manage housing placement activities.
