Job summary

Contra Costa Health Services, through the Contra Costa Adult Continuum of Services (CCACS), seeks a housing case manager to support homeless adults with assessments, benefits enrollment, and service coordination. You will collaborate with Housing Specialists, Income Specialists, and Behavioral Health teams to help clients obtain identification, navigate programs, and build independent living skills.

Qualifications

• AA/AS degree in public health or related field required; BA/BS preferred.
• One year of full-time housing case management or equivalent in a social services or mental health setting serving homeless persons, OR three years experience with a high school diploma or GED.

Responsibilities

• Maintain a caseload and conduct Needs Assessment interviews (psychosocial, health, housing, income, AOD, legal).
• Meet clients weekly to develop and monitor case/life/treatment plans.
• Assist clients in obtaining identification and benefits (Medi-Cal, SSI, GA, CalFresh) at entry.
• Coordinate referrals to Housing, Income, Mental Health, and other services.
• Educate clients on daily living skills to improve self-sufficiency and build rapport.
• Update HMIS records and submit forms within 24 hours of service.
• Attend required meetings and trainings.
• Complete Intake for new clients and assist in income planning and job resources.
