Job summary

Hope Solutions is seeking a Case Manager to deliver home-based case management and ongoing assessment for housing stability in Contra Costa County. You will link clients to medical, mental health, recovery, financial, and legal services and support landlord/tenant relations to prevent eviction.

The role requires a trauma-informed, strength-based approach, collaboration with Housing Authority and Health Housing staff, and mobile, community-based service delivery.

Qualifications

• BA degree in social work or related field; equivalent experience considered.
• Minimum of two years’ experience in human services, preferably with housing retention barriers such as substance abuse, mental health, and/or income challenges.
• Bi-lingual in English/Spanish preferred.
• Must have a valid California driver’s license and be willing to drive for work.
• Must pass LiveScan screening and TB test.

Responsibilities

• Provide home-based case management and ongoing assessment of consumer needs.
• Link clients to medical, mental health, recovery, financial, and legal services needed for housing stability.
• Assist with budgeting, benefits access, and housing/landlord relations.
• Deliver services in the community and client homes with trauma-informed, strength-based approach.
