Job summary

SHELTER, Inc. is seeking a dedicated Case Manager I in Concord, CA to support shelter participants and guide them toward housing stability. You will conduct intake, assess needs, and coordinate resources to empower families and individuals to achieve self-sufficiency.

Responsibilities include managing a caseload, developing collaborative care plans, documenting progress in HMIS, and referring participants to community resources while maintaining confidentiality and ethical standards.

Qualifications

• Associate degree in social services or related field required or equivalent experience.
• Minimum 2+ years of direct Case Management experience with knowledge of public benefits and social services systems.
• Experience with low-income or homeless populations and trauma-informed care is preferred.

Responsibilities

• Intake and eligibility screening; information on services and required consents.
• Complete assessments (HMIS Intake, VI-SPDAT, Housing Retention) to determine strengths and needs.
• Manage caseload of 15–20 shelter participants and develop customized plans with goals and services.
• Coordinate with staff and partners; ensure follow-up and continuity of care.
• Ensure confidentiality, release of information, consent, and mandated reporting requirements.
