Job summary

LifeMoves in Redwood City, CA is seeking a dedicated Case Manager to support clients moving toward safe, stable housing. You will develop individualized case plans, connect clients with housing, benefits, healthcare, and community resources, and document outcomes in HMIS.

You will work in a trauma-informed, strength-based framework, coordinating with Kaiser teams, colleagues, and partners, and participating in trainings, supervision, and occasional evenings or weekends as schedule requires.

Qualifications

• Experience in case management and supportive housing services.
• Trauma-informed approach with strong communication and empathy.
• Proficiency with documentation and data systems (HMIS).

Responsibilities

• Develop and maintain individualized case plans focused on housing, employment, benefits, and well-being.
• Connect clients to housing searches, benefits enrollment, healthcare, and community resources.
• Coordinate with partners and transport services to support client access.
• Maintain accurate client documentation and track outcomes in HMIS and agency systems.
• Advocate for client needs and rights when working with landlords, service agencies, and community partners.
