Job summary

SPECIAL SERVICE FOR GROUPS is seeking a Bringing Families Home Case Manager in Los Angeles to support housing placement and stabilization for families through the Coordinated Entry System. Under the supervision of the Program Manager, you will manage eligibility screening, service coordination, and thorough case documentation.

Qualifications

• Some college courses or related admin/housing field required.
• Minimum two years job-related experience working with homeless individuals and families OR five years in social services; case management experience preferred.
• Experience with HMIS/CS Care and documentation standards (HIPAA) is preferred.

Responsibilities

• Coordinate housing placement and supportive services to help families transition to permanent housing.
• Create/update participant records in HMIS/CS Care and track housing status.
• Enter data and maintain accurate case notes within 24 hours of service delivery.
• Prepare and submit weekly reports and productivity metrics.
• Assist clients with housing plan development and budgeting to sustain housing after subsidy.
• Attend meetings and collaborate with housing partners and funders.
