Responsibilities

• Conduct intake and assess participant needs to develop ISP.
• Provide referrals and support to resources to achieve ISP goals.
• Complete program assessments (e.g., VI-SPDAT).
• Monitor progress with 90-day reviews and adjust plans as needed.
• Maintain accurate documentation and confidentiality per HIPAA.
• Deliver crisis intervention and trauma-informed care as appropriate.
• Use motivational interviewing to support clients and connect to services.
