Job summary

Jericho Project is seeking a Case Manager for Loring Place, a 52-unit Permanent Supportive Housing program in the Bronx. You will serve adults with mental illnesses and substance use histories, with weekly to monthly contact with residents and colleagues. Compensation ranges from $53,000 to $55,000 annually.

The role requires coordination with psychiatrists, referrals to benefits, and ongoing service planning. Strong organizational and communication skills are essential.

Qualifications

  • A Bachelor's degree in human services or related field is required.
  • Experience working with homelessness, substance use, mental health and/or trauma settings preferred.
  • Must be willing to work a weekend day.
  • Must possess strong organizational, writing, communication, and computer skills.
  • Candidates must be self-starters with the skills and energy to work with high-need individuals.
  • Bilingual English/Spanish strongly preferred.

Responsibilities

  • Maintain a caseload of high risk/high-intensity tenants.
  • Provide enhanced case management using Housing First principles.
  • Conduct assessments and program intakes for residents.
  • Ensure data accuracy and complete documentation.
  • Develop and update person-centered service plans every 6 months.
  • Coordinate with psychiatry for appointments and liaise with medical staff.
  • Provide referrals and linkages to public benefits including healthcare and SNAP.
